Please read these instructions carefully before completing the form.

·       Submitting this form does not ensure that we can offer you a group project. Our staff will contact you within a week via email to complete the request process and confirm the details. If we cannot place your group, we will share your request form with other city departments to learn if they can find placement. 

·       Please request only one activity per form submission. Complete a separate form for each activity.

·       Maximum group size: 30 people 

·       Fields marked with an “*” are required. Enter “N/A” if appropriate.

Cancellations: Please note that if the project is cancelled due to snow, wind, rain, or colder weather, we do not have indoor projects as an option.
